What is the description field under symptoms for?

The description field on the symptom table on the treatment plan tab, allows clinicians to extend on the behavioral observations captured in the symptom name field. Often it is a record of the intensity, frequency, duration, or adaptive impairments created in the patient's life by the symptom. This may mean individualizing the data, quantifying features, or highlighting how the symptom interferes with functioning. So for depressed mood it might say “feels unhappy 5 of 7 days per week” or “scores 32 on the Beck Depression Inventory” or “calls in sick for work due to depression at least two days per week.”
