What is the description field under Treatment Plan for?

The description field in the goals table allows clinicians to tailor the language of the goal to specify what any ambiguous statements may mean. If a clinician wrote a goal that stated Jane will show a reduction in fire setting behaviors, the problem description may read "Jane sets things on fire at least twice per week." to reflect the baseline to measure the improvement against. Alternatively, you may use the description of the problem to record patient language regarding the problem that they want assistance with, while the goal is written in more formal terms. For example, you may write a goal that John will demonstrate appropriate self-care and pro-social behaviors. Under problem it may be recorded that John reports he has not left his apartment in two weeks.
