How should I gauge frequency?

From TherapyCharts

Frequency is directed by the acuity of the patient’s illness and indications of what level of care is needed to keep the patient safe and to effectively monitor progress towards the established objectives. It refers to the clinician's direct interventions that will occur: sessions, phone consultations, homework assignments, journal activities, etc. Thus, for a patient who meets monthly with the clinician and does not have any assigned work between sessions the frequency would be monthly. For a patient who meets with the clinician once per week and is assigned to complete a behaviour log daily the frequency would be daily. When you increase the frequency, please be advised that you must document progress at that level. So for the second patient if they only completed the log two days of seven their compliance with treatment would be low and the narrative would reflect five missed interventions.
