Acute Intoxication and Withdrawal

  • For substance abuse patients, clinicians will typically leave a note in the narrative to see the risk section of the Progress Note template for risk related to relapse, intoxication, or medical complications due to withdrawal. It is then easy to rate the risk from low to high by simply selecting the appropriate box.

Bio-Medical Conditions and Complications

  • These concerns are recorded as Axis III diagnoses by selecting the appropriate bio-medical condition from the ICD-9 during treatment planning. Some clinicians will additionally list symptoms associated with these conditions in the symptom field to be able to update whether it has been maintained, improved, or deteriorated between sessions.

Cognitive, Behavioral, and Emotional Conditions

  • Are listed explicitly during treatment planning as an objective. Objectives related to substance use can be found in the Addiction Treatment Planner Group. Clinicians then can rate the patient's progress from 0 to 100% complete on the progress bar in the objective table of each progress note.

Readiness / Motivation

  • Many clinicians utilize the Patient compliance section to record information related to this dimension. Clinicians who wish to use more detailed language can access the Substance Abuse Progress Note planner for language discussing motivation in the narrative.

  • Items related to continued problem areas and newly gained skills or knowledge are generally listed explicitly during treatment planning as an objective. Objectives related to substance use can be found in the Addiction Treatment Planner Group. Clinicians then can rate the patient's progress from 0 to 100% complete on the progress bar in the objective table of each progress note.

Recovery Environment

  • Environmental concerns are generally recorded on the Treatment plan under either client strengths for those with adequate to supportive social environments or through added objectives to improve these relationships. Clinicians then can rate the patient's progress from 0 to 100% complete on the progress bar in the objective table of each progress note.
