Creation date and signature date are legally defined fields. While some systems keep this information only in the required database, TherapyCharts chooses to openly share it with the user so that each clinician is aware of the meta data that is collected on each patient and can take steps within their practice to create policies that conform with state, federal, and third party requirements which can vary. While you can always change the date of service fields, the time stamp dates are logged when you click "New" for a blank document within a patient's file and when you enter your PIN to save a document as the official medical record. If you have extenuating circumstances and the creation or signing of a note is delayed, please feel free to enter a statement into the narrative portion of the document describing what has happened. If you are entering historic data from paper archives, consider adding a custom field with a name like "Date from original document" to reflect an accurate history of your charting.
