What are the signifiers?

The signifiers allow clinicians to mark the primary diagnosis for insurance billing purposes as well as to highlight what the focus on therapy will be during this treatment episode. Others in the list allow the clinician to mark diagnosis that may be being considered and need further observations (provisional), those diagnosed by previous therapists (by history), or to demark other diagnoses being considered (rule out).
