= Canceling an Account = &lt;br /&gt;
Canceling an account is designed for customers who do not want TherapyCharts to maintain their data anymore and do not want to continue the services.&lt;br /&gt;
&lt;br /&gt;
== Account Status After Cancellation ==&lt;br /&gt;
&lt;br /&gt;
After your account is canceled, the data in your account will remain accessible in a read-only format for 30 days. During that 30 day period you will still be able to login and print your data. After 30 days you will no longer be able to login and your data will not be accessible again.&lt;br /&gt;

== What happens to my records after 30 days? ==&lt;br /&gt;
&lt;br /&gt;
Once an account is canceled, after 30 days, the records will be inaccessible and may be removed from our systems at TherapyCharts sole discretion.&lt;br /&gt;

== Procedure to cancel an account ==&lt;br /&gt;
&lt;br /&gt;
* Log into your TherapyCharts account.&lt;br /&gt;
* Goto Configuration -&amp;gt; User Management.&lt;br /&gt;
* Highlight the user you wish to cancel in the User List.&lt;br /&gt;
* Click the Cancel User button.&lt;br /&gt;
* Confirm you understand and agree.&lt;br /&gt;
* The user will be canceled.&lt;br /&gt;
&lt;br /&gt;
Note: If you are canceling a whole organization, please cancel yourself last.&lt;br /&gt;
&lt;br /&gt;
Note: The user manager is only available for users with the appropriate permission, so please use one of those users to cancel your account(s).&lt;br /&gt;

&amp;#039;&amp;#039;Alternative&amp;#039;&amp;#039;: If the cancel user button does not show on the Conf-&amp;gt;User Management tab: Submit a &amp;lt;b&amp;gt;feedback message&amp;lt;/b&amp;gt; requesting cancellation. If you would like, you can tell us why so we can try to get better. The customer service staff will process the cancel request. The account will be canceled effective the day you submit the request even if it is processed at a later date. If billing occurred after the time the &amp;lt;b&amp;gt;feedback message&amp;lt;/b&amp;gt; request is submitted and the time it is processed by the customer service representative then TherapyCharts will issue a refund electronically via the credit card processor. If billing that occurred before the &amp;lt;b&amp;gt;feedback message&amp;lt;/b&amp;gt; request to cancel, is not eligible for refund. If you wish to cancel effective a date in the future please be sure to include that date in your initial request.&lt;br /&gt;

== Ways we will NOT take cancellations ==&lt;br /&gt;

We do &amp;lt;i&amp;gt;not&amp;lt;/i&amp;gt; take cancellations over the phone, email, or through the regular web forms.&lt;br /&gt;

We must have an authenticated request tied back to the account to process it. This is also true for any other account modification that has the potential to change access to the records. We do this for security reasons in order to prevent anyone from impersonating our clinicians, canceling our clinicians accounts, and preventing access to the EHR records.&lt;br /&gt;
&lt;br /&gt;
Additionally, customer service representatives that answer the phones, look at emails, and process the public web question forms do not have access to the elements of the system that allow them to perform a cancellation.&lt;br /&gt;
&lt;br /&gt;
Any cancel requests sent via phone, email, or web forms will NOT be canceled, will be ignored, and no response given. Please submit an internal feedback message.&lt;br /&gt;

= Creating a New Progress Note =&lt;br /&gt;

To create a new progress note, just click the new button (with the green icon) on the progress note located in the upper right hand corner. Each progress note copies from the last version so you data entered in the first note will appear in the next version of the patient&amp;#039;s notes and then just be updated from there. This prevents the need to flip through the notes to easily see what was worked on in the last session. Most of the information on the progress note is stable and our users have told us that the most commonly worked on area is the narrative field, which can either be used with template language to make updates easy or highlighted and typed over each session.&lt;br /&gt;
&lt;br /&gt;
If you believe that you will want to modify the note you simply click “save draft.” You may edit progress notes that are saved as drafts. If the note is complete you would click “sign and save” enter your electronic signing pin in the dialog box and click submit. Once a note is signed it may not be edited. If changes are needed click “new” in the upper left hand corner to get a copy of the old progress note than you can make any changes and document the addendum.&lt;br /&gt;
&lt;br /&gt;
TherapyCharts allows mental health workers to sign various documents using a signing pin. This signing pin helps ensure the authenticity, integrity, and security of the electronic signature. As a student, your notes will not be completed and entered into the medical record until your supervisor signs them. You will not be able to create a new note in this file until both signatures are added.&lt;br /&gt;

If you have not signed the current progress note, then the New button will be greyed out. In order to create a new note you must have signed the current note. Once signed the New button will be clickable.&lt;br /&gt;

If you are part of a organization, then your organization may not have granted you permission to make new progress notes. This is common for support staff.&lt;br /&gt;

&lt;div&gt;TherapyCharts is working toward integration with North Carolina HealthConnex (NC HC). The state of North Carolina is backlogged. We discussed the process with them at the end of March 2019. The process is straightforward and is initiated by the therapist practice (not by the TherapyCharts).&lt;br /&gt;
&lt;br /&gt;
= Background =&lt;br /&gt;
&lt;br /&gt;
The state of North Carolina is requiring therapist to submit certain pieces of reporting data to the start electronically. This is called the North Carolina Health Connex. Therapists in NC have until the end of May 2019 to be compliant. (To see if your practice is required to participate or for more info see https://hiea.nc.gov/frequently-asked-questions)&lt;br /&gt;
&lt;br /&gt;
 &lt;br /&gt;
= Process = &lt;br /&gt;
&lt;br /&gt;
First, the practice submits the NC HC paperwork. When the practice submits the paperwork, the practice list TherapyCharts as your EHR provider for reporting only compliance (not bi-directional). When NC HC process the submitted paperwork, this triggers the NC HC internal on-boarding process.&lt;br /&gt;
&lt;br /&gt;
Next, the NC HC team will contact TherapyCharts about 3-6 months later. We will then have about one year to work on integration to get the data transfer to North Carolina. During that time a practice will be considered in compliance even no data is sent to them by the end of May 2019.&lt;br /&gt;
&lt;br /&gt;
Unfortunately, NC HC wouldn&amp;#039;t share actionable technical information, let TherapyCharts tech members speak to any NC HC technical team members, nor their integration partner, as they are completely backlogged. However, TherapyCharts believes that once we get the specs will be able to do this integration project once we have been contacted by the North Carolina HealthConnex and go through their vending process.&lt;br /&gt;
&lt;br /&gt;
The whole process is triggered by the practices submitting the paper -- they explicitly stated an EHR vendor _cannot_ initiate the process ourselves. If you have any questions both Kenya and Jessica at NC HC can be contacted to help.&lt;br /&gt;

= Compliance= &lt;br /&gt;

This is most likely caused by a browser related issues such as:&lt;br /&gt;
* (a) corruption of cookies or cached data;&lt;br /&gt;
* (b) browser plugins or extensions; or &lt;br /&gt;
* (c) super slow internet connection (dial-up speed).&lt;br /&gt;
&lt;br /&gt;
==Solution==&lt;br /&gt;
&lt;br /&gt;
You must be using Google Chrome. Other browsers have similar features but the process is different.&lt;br /&gt;
&lt;br /&gt;
===PART I: Verify the login is successful, but the main tab doesn&amp;#039;t load===&lt;br /&gt;
* (1) close all google chrome windows and restart google chrome with open one window&lt;br /&gt;
* (2) goto secure.TherapyCharts.com&lt;br /&gt;
* (3) enter your login information&lt;br /&gt;
* (4) at the paper flipping screen, wait 60 seconds, if the paper flipping screen remain longer then 60 seconds, your browser is a candidate for this issue&lt;br /&gt;
* (5) hit F5 to force google chrome to reload the page a 2nd time, the flipping papers should reappear, &lt;br /&gt;
* (5a) in 30% of the cases you will login to the main tab at this point&lt;br /&gt;
* (5b) in the rest of the cases, proceed to part II below&lt;br /&gt;
&lt;br /&gt;
===PART II: Verify login does work in a Google Chrome Incognito Window===&lt;br /&gt;
* (1) close all google chrome window and restart google chrome with one window open&lt;br /&gt;
* (2) Open a Incognito Window via the main menu (or the keyboard comment CTRL+SHIFT+N&lt;br /&gt;
* (3) In the Incognito Window, goto secure.TherapyCharts.com&lt;br /&gt;
* (4) login, the papers should paper, then shortly you should be at the main tab&lt;br /&gt;
* (a) If the main tab appears then this confirms it is something corrupted with cookies or cached data OR you have a plugin or extension that is breaking things; Proceed to PART III&lt;br /&gt;
* (b) If the paper continue to be present even in the Incognito Window contact customer support and have your issue escalated to the technical support team &lt;br /&gt;
&lt;br /&gt;
===PART III: Clear Old Data===&lt;br /&gt;
* (1) Open Clear Browser Data Dialog Box (from the menu) or open it wit the keyboard command CTRL+SHIFT+DEL&lt;br /&gt;
* (2) Choose Last 7 days&lt;br /&gt;
* (3) Choose Cookies and other site data&lt;br /&gt;
* (4) Choose Cached images and files&lt;br /&gt;
* (5) It is ok to unselect &amp;quot;Passwords&amp;quot; and &amp;quot;Autofill form data&amp;quot; if you use the browser for other sites&lt;br /&gt;
* (6) Click the Clear data button in the dialog box&lt;br /&gt;

Because of the encryption employed we cannot reset your password. DO NOT leave it near your computer or out in the open. == Requirements To Export Data ==&lt;br /&gt;

In order to use, the following must be met:&lt;br /&gt;
* your account (all users and organization) must be in good standing;&lt;br /&gt;
* your organization must support the functionality;&lt;br /&gt;
* your user must have permission within the organization (be an administrator account and be able to see everything);&lt;br /&gt;
* you must agree to the requirements and terms shown in the application on the screen during the process.&lt;br /&gt;

A non-exhaustive list of potential items that could stop data export include but is not limited to:&lt;br /&gt;
* any user within your organization is suspended;&lt;br /&gt;

For example, if you have a progress note open when the internet connection stops working then you can finish typing that note. After the connection comes back, just click save. (There is an lost connection indicator that will appear in the lower right corner over the TherapyCharts graph when the connection is lost.) However, there is no way to access the TherapyCharts system if your internet connection goes down for a extended period of time.&lt;br /&gt;

No. Electronic signatures are governed by two primary laws. On October 16, 2000, Michigan enacted the Uniform Electronic Transactions Act (UETA), which is based on a uniform act promulgated by the National Conference of Commissioners on Uniform State Laws (NCCUSL). UETA is also adopted in 47 other states. Both laws were intended to smooth the way for conducting business electronically by giving validity to electronic records and signatures.&lt;br /&gt;

At the transport layer when the data is moving from your computers to our servers is handed by the web browser using the HTTPS standard. And occasionally certain pages that work fine before just refuse to load on the iPad. We have investigated this odd behavior and believe it to be an issue with the iPad device itself.&lt;br /&gt;

Yes, as long as you know your current password you can change it to something new. Make sure your new password is solid. Sometimes. Some virus scanners, popup blowers, and web browser setups, in particular IE, causes problems. Please note we strongly recommend you use Chrome and not IE. The common symptom of this is when you try to login you find you get stuck on the animated loading screen.&lt;br /&gt;

We sent our computer geeks to study scanned documents for a week. They determined that the average size is 52kb per page when scanned at our recommend settings. Our recommended settings are based on 8.5&amp;quot;x11&amp;quot; paper at 200 DPI black and white compressed using TIFF Group IV -- these setting are great for text document storage, similar to the quality of a fax machine, and configurable on most scanners. Our studies have shown when documents are scanned to this level they are ideal for EHR storage because they are small and compact and retrieved quickly. The default limit for storage is 1024MB which gives room for about 20,000 pages of scanned text. There is the ability to purchase more space as the organization needs because you need more pages or choose to scanned pages at different size, bit depth, # of colors, and compress type then our recommendations.&lt;br /&gt;

In the meantime, they can&amp;#039;t be read by anyone because they are encrypted and only your username and password can decrypt them -- not even our staff can read them. So if you do leave DO NOT FORGET OR LOSE YOUR PASSWORD, because if you do you won&amp;#039;t get your old records back later. During this time or before deactivation, you can print all the records for a given patient by going to Patient Info Tab and selecting Print All Clinical Records. Canceling an account is designed for customers who do not want TherapyCharts to maintain their data anymore and do not want to continue the services.&lt;br /&gt;

If you would like, you can tell us why so we can try to get better. The customer service staff will process the cancel request. The account will be canceled effective the day you submit the request even if it is processed at a later date. If billing occurred after the time the &amp;lt;b&amp;gt;feedback message&amp;lt;/b&amp;gt; request is submitted and the time it is processed by the customer service representative then TherapyCharts will issue a refund electronically via the credit card processor. These can be used if you need to produce the records for examination by the court or if you want to print them to refresh your memory while you might be given testimony on an issue before a count. Once generated they download to your local machine as a PDF file which can then be printed.&lt;br /&gt;

&lt;br /&gt;
IF a customer wishes to submit their own BAA for our legal team to review&lt;br /&gt;
THEN please send a feedback message requesting a &amp;quot;Customer Supplied BAA Legal Review&amp;quot;.&lt;br /&gt;
&lt;br /&gt;
The following overall steps will occur:&lt;br /&gt;
* You will be emailed the TherapyCharts Supplemental BAA for your legal team to review;&lt;br /&gt;
* After review you may submit a proposed agreement and present the reason you believe you qualify to be special and do something different then everyone else;&lt;br /&gt;
* A $550 legal review fee assessed against your billing plan (this fee does not imply the legal team will approve the document submitted);&lt;br /&gt;
* The legal team will review the submitted document to determine how the provisions differ from those on our document. This usually takes 30 days to do an initial review a document.&lt;br /&gt;
&lt;br /&gt;
IF the document submitted contains either:&lt;br /&gt;
&lt;br /&gt;
(a) any terms or obligations differing from those found on the TherapyCharts document; or &lt;br /&gt;
&lt;br /&gt;
(b) materially differs from the sample documents found online at https://www.hhs.gov/hipaa/for-professionals/covered-entities/sample-business-associate-agreement-provisions/index.html, including but not limited to indemnification, changes in notification times, specificity, or definitions;&lt;br /&gt;
&lt;br /&gt;
THEN the submitted document will likely be rejected.&lt;br /&gt;
&lt;br /&gt;
It is also possible that the legal team will request a meeting with your attorney to further understand the differences and the reason your organization believes it should be treated differently. It is highly recommended that your attorney be prepared to discuss your organizations risk management, policies and procedures, cyber security, and operations; and why they require a change from our standard document.  &lt;br /&gt;
&lt;br /&gt;
In the end, usually within 3 months, the legal team, at its sole subjective discretion, will have the submitted document either:&lt;br /&gt;
* rejected;&lt;br /&gt;
* approved with modifications based on the discussion with your attorney; or&lt;br /&gt;
* approved with an additional significant periodic &amp;quot;Special BAA fee&amp;quot; billed to your account on a periodic basis for the duration of the account.&lt;br /&gt;

&lt;div&gt;TherapyCharts saves bandwidth by instructing your web browser to store many parts of the site in its local cache (normally on your hard drive), so that they are downloaded only once.  This includes the layout, images previously displayed, style sheets, JavaScript, etc.&lt;br /&gt;

Sometimes, this has the undesired—and highly confusing—result that a recent change appears to be ignored.  There may be other abnormalities, such as changes to the site&amp;#039;s interface or changed user preferences having no effect.&lt;br /&gt;
