== Setting up accounts for multiple signatures ==&lt;br /&gt;
There are times when an organization may require more than one signature for clinical documentation. Often this falls into either a supervision requirement, state medicare requirements, or as procedural requirements for the organization. To set up multiple signatures on documentation:&lt;br /&gt;
&lt;br /&gt;
The administrator would login to their account. Navigate to the &amp;quot;Configuration&amp;quot; area. In the second navigation row, select the &amp;quot;User Management option. Scroll to the Staff Role table at the bottom of the tab. By default there are several staff roles, students are the only role that are initially set up to require two signatures. You may modify signing permissions of any group to add or remove signature requirements.&lt;br /&gt;
&lt;br /&gt;
If the situation is for supervision then the default settings are likely sufficient. Simply assign users who will be supervising and signing off on other staff&amp;#039;s notes to the &amp;quot;Supervisor&amp;quot; group. Then assign any staff requiring supervision to the &amp;quot;Student&amp;quot; group. Supervisor&amp;#039;s will have the ability to view, sign, or request edits on documentation created by the student.&lt;br /&gt;
&lt;br /&gt;
If the situation is for state medicare requirements, you may require a physicians signature on all or selected documentation. Clinics in this situation generally add signature requirements to the &amp;quot;Therapist&amp;quot; role consistent with the regulations. The supervising physician then is either assigned to the &amp;quot;supervisor&amp;quot; group or has an additional role created in the table with the appropriate permissions.&lt;br /&gt;
&lt;br /&gt;
Depending on your organization&amp;#039;s needs you may need to create additional staff roles to accommodate signing requirements. These allow you to categorize staff according to the specific permissions they need and the signature relationships between hierarchical relationships. You may have up to twelve distinct staff roles within the organization. If you require more, please contact our technology team for assistance.&lt;br /&gt;

== How Notes are finalized ==&lt;br /&gt;
The requirements for any documentation are established when it is created and are displayed along the system timestamps along the top. When two signatures are required the creator of the note will see a &amp;quot;Finalize&amp;quot; in place of the usual &amp;quot;sign and save.&amp;quot; Once they have entered their signature the file will appear in the patient list of supervisors when the filters are set to &amp;quot;Others (action required). The supervisor may then review the note. The TherapyCharts system allows you to add additional fields to the clinical documentation tabs to record addition information. We call this a Custom Field.&lt;br /&gt;
&lt;br /&gt;
A custom field has two parts:&lt;br /&gt;
(1) the custom field name; and&lt;br /&gt;
(2) the custom field value.&lt;br /&gt;
&lt;br /&gt;
= What type of custom fields are available? =&lt;br /&gt;

At this time we allow only plain text custom fields. The name of the field should be less then 64 chars in length. Each name must be unique. The string you place in the value should be less then 128 chars in length.&lt;br /&gt;

= How many can I have on a form? =&lt;br /&gt;

There is a limit of 20 custom fields per form.&lt;br /&gt;

When you are looking at the form, there will be an expandable area called &amp;quot;Custom Field&amp;quot;. Once expanded this will present a grid area with an Add, Edit, and Delete button. The custom fields attached to a form will be made read only when you sign the form. Similarly when you make a new version of a form it will copy the data from the previous version into the new version.&lt;br /&gt;

No problem, under the Configuration tab -&amp;gt; General there is a organization wide Custom Field Editor. In this editor you can configure which custom fields appear on which forms and their names. You can also configure the add behavior:&lt;br /&gt;
(a) On Patient Creation - this will add the custom field to the first document, if you later delete it, it will not be added again with each new version.&lt;br /&gt;
(b) On New Version -  this will add the custom field list to the document every time you create a new version of the document.&lt;/div&gt;</summary>

If you are not using TheraScribe 5, and instead are using another product, then please upload an export of your data from your existing EHR to the document manager tab within TherapyCharts. Then using the feedback button submit a request for import analysis. Turn around time varies in analysis but usually takes 2-4 weeks. Analysis means that the TherapyCharts tech team will tell if it is even possible to do an import and an estimated timeline and cost to do so.&lt;br /&gt;
&lt;br /&gt;
= How to do it =&lt;br /&gt;
The TheraScribe 5 desktop based product stored your patient information in a file on your local machine. That filename ends in .MDB and is located where you choose to install it when you first installed TheraScribe 5. Common locations include in your Documents folder.&lt;br /&gt;
&lt;br /&gt;
When the TheraScribe 5 import tool is available for your account, it will be located under the Configuration -&amp;gt; Document -&amp;gt; Import section. From there you will be able to open an upload tool which will allow you to select the .MDB file from your local filesystem. Once you select the MDB file, you can upload it to the system and our import tool will process it.&lt;br /&gt;
&lt;br /&gt;
= What is imported? = &lt;br /&gt;
In TheraScribe 5, there is a left navigation area containing primary and secondary links. The data from the first two primary links will be imported. Those areas are titled: Personal Data, and Assessment. The functionality to import Personal Data will be released first, and the Assessment import functionality will come a number of weeks after.&lt;br /&gt;
&lt;br /&gt;
Personal Data contains the following sub-sections: Demographics, Provider, Insurance, General Notes, HIPAA&lt;br /&gt;
Assessments contains the following sub-sections: Psychosocial History, Strengths / Weaknesses, Assessments Given, Mental Status, Recovery, Summary.&lt;br /&gt;
&lt;br /&gt;
The fields in each of these sub-sections will be imported into the analogous fields in TherapyCharts, spread across primarily the Patient Info, Intake, Documents, and Treatment Plans tabs.&lt;br /&gt;
&lt;br /&gt;
The time between .MDB file upload and import may be 1-2 weeks.&lt;br /&gt;
&lt;br /&gt;
= Under which user account (clinician) do my clients (patients) go? =&lt;br /&gt;

When the importer runs, it will place each client-patient into the owning user-clinician as found in the TheraScribe database. To determine a match between the TheraScribe user and the TherapyCharts user the importer looks at the email address in both. A match occurs &amp;lt;i&amp;gt;only if both emails are identical&amp;lt;/i&amp;gt;. For any non-match, the import will first creates TherapyCharts clinician user accounts for all users found in the TheraScribe database (that are not already created in TherapyCharts). This accounts are marked as special imported accounts and are set as non-billable. Then the clients-patients will be imported. Please be sure you have set an email address for all your TheraScribe users that match the corresponding TherapyCharts user you wish to have the clients-patients imported under. Failure to do so will result in those clients-patients being imported separately and you having assign them to a primary therapist by hand.&lt;br /&gt;
&lt;br /&gt;
= What about my Treatment Plan and Progress Notes? =&lt;br /&gt;
Those are imported too. Because there are differences between TheraScribe and TherapyCharts there are differences in how they look on the screen. Our team may revisit importing additional information in the summer based on the feedback from the TherapyCharts customers who have used the MDB import feature.&lt;br /&gt;

TherapyCharts allows mental health workers to sign various documents using a signing pin. This signing pin helps ensure the authenticity, integrity, and security of the electronic signature.&lt;br /&gt;

No. Electronic signatures are governed by two primary laws. The federal Electronic Signatures in Global and National Commerce Act (E-Sign) was signed into law on June 30, 2000 and became effective, at least for the most part, on October 20, 2000. On October 16, 2000, Michigan enacted the Uniform Electronic Transactions Act (UETA), which is based on a uniform act promulgated by the National Conference of Commissioners on Uniform State Laws (NCCUSL). UETA is also adopted in 47 other states. Both laws were intended to smooth the way for conducting business electronically by giving validity to electronic records and signatures.&lt;br /&gt;

Both E-Sign and UETA provide that an electronic record may be used to satisfy a record retention requirement that is imposed under another law, (such as HIPAA) so long as the electronic record accurately sets forth the information in the record and remains accessible to all persons who are entitled to access the record. Once a note is signed-and-saved it appears as a previous version. There are two ways to access the previous versions.&lt;br /&gt;

After selecting the patient you want click the History tab. In the history tab you will see a list of all previously signed-and-saved versions of all your clinical documents. Clicking the icon in the row will load the particular version.&lt;br /&gt;

When looking at the current progress note there are four buttons to the right side of the icon bar (which is the bar just below the tabs Progress Note, Treatmen Plan, etc at the top of the web browser window). The four button are also located just to the right of the current patient name which is also shown in the icon bar. The four buttons are left to right:&lt;br /&gt;
goto first note&lt;br /&gt;
goto previous note&lt;br /&gt;
goto next note&lt;br /&gt;
goto last note&lt;br /&gt;
So if you can imagine a stack of paper with version 5 of a progress note on the top clicking the previous note button will take you to version 4 or the one under it.&lt;/div&gt;</summary>
