The patient list by default will only show your assigned patients for your convenience. To view other clinical records you must add the other clinician&amp;#039;s patients to your patient list. To do this, use the filter in the lower left hand corner of the patient list &amp;quot;+/- Filter&amp;quot; it is represented by a meeple in a blue shirt. The list will allow you to select Mine (patients assigned to your care), Others (patients assigned to other users in your organization), or Mine &amp;amp; Others (all patients in the organization). There are also options to show patients who are no longer active. Once the patients are loaded onto the patient list tab you can navigate through their chart like your own patients. Fill out the form in its entirety. Fill out every field. If your staff does not have a license, enter their job title. If your staff does not have any credentials, enter NA. There are two important trends in psychotherapy treatment planning: (1) Evidence-Based Treatment and; (2) Person-Centered Recovery approach. In some practice settings one or both of these psychotherapy methods are mandated. Wiley’s Treatment Planner books, part of the PracticePlanner Series have copious, high-quality content that addresses both of these trends.&lt;br /&gt;
&lt;br /&gt;
Evidence-Based Treatment: &lt;br /&gt;
&lt;br /&gt;
The use of evidence-based treatment (EBT) is becoming the standard of care in mental health counseling (Kazdin, 2008). EBT has been endorsed, among others, by the American Psychological Association, the American Psychiatric Association, National Association of Social Workers, The American Counseling Association, The Association for Addiction Professionals, and the National Alliance for the Mentally Ill. The American Psychological Association has defined Evidence-Based Practice as, “the integration of the best available research with clinical expertise in the context of patient characteristics, culture, and preferences.” Consistent with this definition we have identified those psychological treatments with the best available supporting research evidence (see Sturmey and Hersen, 2012; Nathan and Gorman, 2015), added objectives and interventions reflecting them in pertinent chapters, and identified these with this symbol: EBT (inside a triangle). Our bestselling Treatment Planner books (Adult, Adolescent, Child, Addiction, Couples, Family, SPMI, Crisis, School, Personality, and Older Adult) have hundreds of EBT Objectives and Interventions suggested for consideration when the practitioner is creating a treatment plan with and for his/her client. These suggestions will remind you of techniques you are probably familiar with and may also present some unfamiliar, evidence-based interventions that you may want to learn more about before you&lt;br /&gt;
discuss with the client integrating them into the treatment plan.&lt;br /&gt;
&lt;br /&gt;
Person-Centered Recovery: &lt;br /&gt;
In its final report entitled Achieving the Promise: Transforming Mental Health Care in America, The President’s New Freedom Commission on Mental Health called for recovery to be the “common, recognized outcome of mental health services” (New Freedom Commission on Mental Health, 2003). To define recovery, the Substance Abuse and Mental Health Services Administration (SAMHSA) within the U.S. Department of Health and Human Services and the Interagency Committee on Disability Research in partnership with six other Federal agencies convened the National Consensus Conference on Mental Health Recovery and Mental Health Systems Transformation (SAMHSA, 2005). Over 110 expert panelists participated including mental health consumers, family members, providers, advocates, researchers, academicians, managed care representatives, accreditation bodies, state and local public officials, and others. From these deliberations, the following consensus statement was “Mental health recovery is a journey of healing and transformation for a person with a mental health problem to be able to live a meaningful life in a community of his or her choice while&lt;br /&gt;
striving to achieve maximum human potential. Recovery is a multi-faceted concept based on the following 10 fundamental elements and guiding principles: 1) Self-direction 2) Strengths-based 3) Individualized and person-centered 4) Peer support 5) Empowerment 6) Respect 7) Holistic 8) Responsibility 9) Nonlinear 10) Hope” The 10 principals of mental health recovery are defined in an Appendix to each of the same bestselling books listed in the paragraph above (with the addition of The Group Therapy Treatment Planner). We have also created a set of Goal, Objective, and Intervention statements that reflect these 10 principles. The psychology literature contains authors who have made a case for the advantages of implementing shared decision making associated with Person-Centered planning (e.g., Drake and Deegan (2009); Drake, Deegan, and Rapp (2012). Some counselors may be mandated to create the client’s treatment plan using a Person-Centered Recovery approach. Adams and Geider (2013) have written about how to engage clients in building and enacting collaborative treatment plans and have summarized the evidence basis that person-&lt;br /&gt;
centered care works. The clinician who desires to insert into the client treatment plan specific statements reflecting a Person-Centered Recovery Model orientation may choose from the menu of items found in the Treatment Planner appendix.&lt;br /&gt;
&lt;br /&gt;
In addition to this list, we believe that many of the Goal, Objective, and Intervention statements found in the chapters reflect a Person-Centered Recovery orientation. For example, our assessment interventions are meant to identify how the problem affects this unique client and the&lt;br /&gt;
strengths that the client brings to the treatment. Additionally, an intervention statement such as, “Assist the client in finding positive, hopeful things in his/her life at the present time” from the Suicide Ideation chapter is evidence that recovery model content permeates items listed&lt;br /&gt;
throughout our chapters. However, if the clinician desires a more focused set of statements directly related to each principle guiding the Person-Centered Recovery model, they can be found in an Appendix for these books. In short, many of the Treatment Planner books have directly addressed Evidence-Based Treatment and Person-Centered Recovery and have content to satisfy this direction for treatment planning. Look for the EBT symbol and find the Recovery appendix for suggested statements to add to the client’s treatment plan.&lt;br /&gt;

The first is considered a rolling start. The rolling start involves the clinic starting their support staff on the new software with all new patients. Clinicians then work with the new system for new patients and the old charting system for existing patients. This is generally selected in clinics where their patient encounters are brief and patient loads turn over quickly. On this plan there are two systems that work in parallel to document patient data until the patient load has transitioned to the new electronic system and no paper charts are still in circulation. If patients return to therapy their data (treatment plans, psychosocial evaluations) may be uploaded into the new system from their historic charts to allow for continuity of care. This has the least labor diverted to clinicians and support staff, but can introduce confusion as there are two systems in play until the new system has replaced the old as patients are converted.&lt;br /&gt;
&lt;br /&gt;
The second is a modified rolling start. In addition to the process described above, to facilitate patients being entered into the electronic charting system more quickly as they come due for either reviews or treatment plan updates clinicians enter them into the system. On this plan old information (intake forms, and last session note) may be uploaded into the system as clinicians who are working with existing clients upload their information to the new system. This is most commonly used where the patient base is largely stable and engaged in long term therapy. This process is typically selected by most single users as it quickly converts patients to one system, without being overwhelmed with entering all patients as described below or the need to maintain two systems as described above.&lt;br /&gt;
&lt;br /&gt;
The last approach is a hard start. This is usually used when clinicians have large caseloads that are either already associated with an electronic charting system and they are migrating data or are part of collaborative care where there may be more than one staff responsible for patient charting. Here, if the staff have a pre-existing electronic record database the clinic will pick a date (usually a Friday) and have the upload started in a scheduled date when there is limited clinical activity. Our staff process the electronic data and have the imported material available for the clinicians the following Monday. When there is no existing electronic database this process requires clinicians or support staff to either upload forms from the old system or input data by hand for each patient at their first appointment after the deadline. This is the most labor intensive option for clinicians and is generally used when there is a dedicated medical records staff or support staff who can lead uploading forms or a relatively small patient caseload.&lt;/div&gt;</summary>

When you select this option, you can group the two charts on the group list tab. Click add new group, it will open a new box. In the box most clinicians will enter a group name like &amp;quot;Jones Couple&amp;quot; to indicate the members. Then click edit membership and add the members of the couple. When you are writing most progress notes you can then navigate from the group list to the progress note which will place a copy of the one note in both members charts. With this set up you then would have the option to add personalized notes to either individual&amp;#039;s file regarding their specific presentation, goals, or progress. This method also allows private sessions if they are deemed appropriate.&lt;br /&gt;

Other clinicians work exclusively on coupled goals. They will not meet individually with members of the couple. They will use the contact fields to record the members of the couple&amp;#039;s contact and personal information. This means that there is only one treatment plan and intake that treats the couple as a single organism with united goals. Click on the new button which appears as a paper with a green plus on the left hand side of the icon menu. The form will load with any data from the most recent treatment plan if there has been one created already for this patient for you as the clinician to update. If this is the first treatment plan clicking new will unlock the fields for editing.&lt;br /&gt;

We support diagnosis using the ICD-10 or DSM-IV and have the full ICD-9 available for historic data.&lt;br /&gt;

To update so that only ICD codes appear as an option for the treatment plan diagnosis an administrator will need to login to their TherapyCharts account. Once logged in, click on configuration in the upper right hand corner. Click on the billing sub-tab in the second navigation row, it will appear towards the middle of the screen. The top table on this tab is the diagnostic preferences table, since Sept 31st the default for this table has been &amp;quot;show me everything&amp;quot; as clinicians have been using all of the ICD-9, ICD-10, and DSM V codes based on their region. Most clinics are now selecting either the ICD-10 international description or the ICD-10 DSM V description based on what their staff have trained under. Once you click the appropriate diagnostic category click save. This will update the treatment plan table to show only the diagnostic codes from the category you selected. Please be advised that this will not change historic diagnosis as the new codes do not map precisely with the old code set and clinical judgement is required in most cases to ensure clinical accuracy. There are three Wiley Planner Wizards available in TherapyCharts. The treatment planner series are available on the treatment plan tab, while the homework planners and progress note planners are available on the Progress Note tab.&lt;br /&gt;

The treatment planner wizard will walk you through creating a treatment plan. Click on the TSPlanner button in the second navigation row of the treatment plan tab. This will open the wizard to the available books. Select the book that is most suited for your patient. You can then click next in the lower right hand corner of the wizard. The problem list will help you narrow down the types of symptoms, objectives, and interventions as you continue. Most clinicians will select only the problem that they plan to address in this treatment episode. TherapyCharts limits the problems to no more than four as the treatment plan represents a contract or prescription for treatment and clinicians are responsible to addressing everything included. The next screen shows you symptoms sorted by problem areas. Select the symptoms that apply to your patient, you will have a chance to edit the language later. The next screen has a bank of goal statements that you may choose from as the measure for when this treatment episode is completed. Patients and their clinicians may choose to focus on one problem now and continue therapy with another problem when appropriate. The next screen has objectives that can assist in reaching therapeutic goals. Some will be marked with EBT to indicate they have empirical research studies supporting their use. The next screen will list possible interventions the clinician may use if they are appropriate for the patient&amp;#039;s care and the clinician is skilled in that area. The next screen lists the diagnoses that are most likely, if you do not see the diagnosis you want you can add it on the main treatment plan later. Clicking submit will load all of your selections into the treatment plan. Be sure to update the symptom descriptions to match your patient and completed the person centered planning areas of strengths and preferences if applicable at your office.&lt;br /&gt;

The Progress Note planner is located on the progress note tab. Click on the TSPlanner button to open the wizard. In the right hand upper corner there is a filter that searches based on your diagnosis, if you would like to see all options available in the books, remove the check. You can select the most appropriate book from the planner drop down list. The problems covered in the book will appear along the left hand navigation bar. Expand any of the problems that your patient may be experiencing to see the associated symptoms. Each symptom has a range of three to eleven acuity statements that will appear in the right hand navigation window. Click on the plus sign to add the statement to your narrative section of your progress note. You will be able to edit these later. You can repeat with multiple problems and symptoms. Click close when you have addressed all of the symptoms covered in the session.&lt;br /&gt;

The Homework planner is located on the progress note tab. Click on the TSHomework button to open the wizard. This wizard works very similarly to the note planner. The main difference is that after selecting the planner and expanding the appropriate problems the list will show the available worksheets. Clicking on any of the titles will give you a brief description of the assignment on the right hand side. You can download these either by assigning them to the patient, which will insert a statement into the narrative that homework was assigned. Or by simply downloading the form without the record being created in the patient chart. The files will download according to your configuration in your internet browser.&lt;/div&gt;</summary>

Under the &amp;quot;Billing&amp;quot; subtab in the configuration area, an administrator can set the diagnosis preferences that your organization uses. Most clinics choose a standard reporting option whether ICD-9, ICD-10, or DSM-V. Some clinics need the table to show all of the options as different insurance providers may require different diagnostic codes. &lt;br /&gt;
&lt;br /&gt;
On this same tab you can establish the service and billing location allows users to add where they provide service or alternatively the billing location that should be associated with claims. &lt;br /&gt;
&lt;br /&gt;
The system will allow administrators to automate the creation of invoices to clinical documentation. These invoices can be set to create an invoice for all clinicians in an organization either when notes are created or when notes are signed. Some clinics allow each clinician to set this preference in their own account. &lt;br /&gt;
&lt;br /&gt;
Invoices may be created individually one for each note related to a service, or batched by a set time frame to record all services provided for a patient in a day, week, or month depending on how the clinic processes patient payments. Choosing any of these batch options will list any services provided in the timeframe as separate transactions on one invoice. &lt;br /&gt;
 &lt;br /&gt;
This is also the area that includes the service code tables for the standard price of service codes within the organization. Once a service code has an established price, any invoices created with the service code will reflect the charge in this table, unless the patient has specialized pricing discussed below.&lt;br /&gt;

== At the Patient Level ==&lt;br /&gt;
On the patient information tab is a similar service code table. This is used primarily when clients have customized billing rates that differ from the organization&amp;#039;s standard rates. This can be due to insurance policies that contract at reduced rates or private pay situations where fees are based on income or other factors. To add a customized service fee simply click the add price button and complete the form. You can edit this price table at any time should situations change with the client.&lt;br /&gt;

Click add charge to open the dialogue box to add the necessary information. Therapists may choose to link to a progress note’s service code that does not already have an attached invoice or create a new service code. They will be asked to select a service location. They may add billing notes to increase clarity for the patient if they choose. Click add to put the transaction on the invoice. &lt;br /&gt;

&lt;br /&gt;
To collect payment information click the add payment button. Therapists should record the transaction date, when the payment was made. The next filed asks for type of payment: &lt;br /&gt;

b) co-pay for patient portions of unpaid claims, &lt;br /&gt;
c) claim for insurance payments, or &lt;br /&gt;
d) adjustments for any reductions made either for a sliding scale or contractual arrangments with payors. &lt;br /&gt;
The payment method can be recorded as well cash, credit, check, or claim. There is a field to record payment tracking information whether it is the tracing number on the check or a transaction id for credit and electronic claim transfers. The payment can be tracked to any service code with outstanding balance. The amount captures the details of the payment and again there is a comment field for any notes the clinician or staff wish to make for clarity. Click add to record the payment.&lt;br /&gt;

In order to be compliant with regulations, on printed invoices there will be indications in the foot notes of any outstanding documentation, to alert patients of possible transactions adjustments until all of the supporting documentation is signed and saved and the invoice is posted. Once an invoice has been posted it is viewed as complete and transaction details cannot be edited. Before you post the invoice be sure all of the details are correct. You may edit any transaction while it is open using the edit transaction button. Payments may still be added to posted invoices.&lt;/div&gt;</summary>

Just saying thanks for modifying the Treatment Planner which now can view the homework to consider assigning or download and assign which adds to the progress note.  Also, the removing of the repeating of the identified symptoms cleaned up the tx plan.  &lt;br /&gt;

TherapyCharts sends the blank homework assignments to your local web brower unencrypted because they do not contain sensitive PHI. Sending it unencrypted is more efficient. However Firefox does not like this and blocks the transfer. This is not a bug in TherapyCharts. This is caused by a security setting in Firefox versions 22, 23, and above. You need to disable this setting.&lt;br /&gt;

Frequency is directed by the acuity of the patient’s illness and indications of what level of care is needed to keep the patient safe and to effectively monitor progress towards the established objectives. It refers to the clinician&amp;#039;s direct interventions that will occur: sessions, phone consultations, homework assignments, journal activities, etc. Thus, for a patient who meets monthly with the clinician and does not have any assigned work between sessions the frequency would be monthly. For a patient who meets with the clinician once per week and is assigned to complete a behaviour log daily the frequency would be daily. When you increase the frequency, please be advised that you must document progress at that level. So for the second patient if they only completed the log two days of seven their compliance with treatment would be low and the narrative would reflect five missed interventions.&lt;/div&gt;</summary>

The TherapyCharts system has a generalized Progress Note form (shown on the progress note tab) to allow one to write any kind of progress note. Since the SOAP note format is just a more structured and formalized progress note, there are two approaches to writing SOAP notes in TherapyCharts. (When we&amp;#039;re talking about SOAP notes we&amp;#039;re talking about these: http://www.coe.ufl.edu/Counselor/PracticumInternship/Files/SOAPNotes.pdf)&lt;br /&gt;
First, simply write the text of your SOAP note as you usually would in the free text &amp;quot;Progress Note&amp;quot; field on the Progress Note form -- done. This approach requires little change in your current process but doesn&amp;#039;t take full advantage of the features of TherapyCharts.&lt;br /&gt;
Alternatively, TherapyCharts encapsulates parts of the SOAP note in other fields. Each Progress Note form (as shown on the progress note tab) contains fields that address those required in a SOAP note. We have a &amp;quot;Progress Note&amp;quot; free text field for subjective or objective data, we integrate the reported symptoms independently with an updateable field for objective data which allows for independent tracking, and we include the treatment plan objectives and medication management for treatment planning.
